September 19-22, 2009: The Omni Dallas Hotel at Park West, Dallas, Texas.
Designated for a maximum of 30 AMA PRA Category 1 CreditsTM of CME. Limited seating, so don’t delay!
Registration: 6:00 - 7:00 p.m. on Sep 18 and 7:00 - 8:00 a.m. on Sep 19. Course ends at noon on Sep 22. For this schedule, we recommend that you arrive by Sep 18 and allow 3 hours from end of course to flight departure on Sep 22.

Recert Course Cancellation/Changes Policy:
Service charges will be deducted from refunds on Course cancellations or changes based on when request is made as follows:
- 30 days or more before Day 1 of the Course—a $100 service charge (may be applied as credit toward a future Course).
- 10-29 days before Day 1 of the Course—a $200 service charge (no future Course credit).
- Less than 10 days before Day 1 of the Course or later—No refund will be given.
Travel and Lodging: Attendees arrange for their own travel and lodging. Reserve your lodging immediately after your course registration is confirmed. The deadline for the specially negotiated room rate of $175/night, which includes a deluxe continental breakfast and full hot lunch, is August 28. Reserve early -- number of rooms is limited.
