CME: MedStudy designates this educational activity for a maximum
of 80 AMA PRA Category 1 CreditsTM. Physicians should only claim credit commensurate

Cancellation Policy: There will be a $100 service charge on all refunds requested
through April 21, 2009 (one month prior to the Intensive) and a $200 service
charge on refunds requested April 22, 2009 through May 20, 2009. No refund requests
will be honored after May 20, 2009.
